Overview

SARATHI is a purpose-built healthcare platform designed exclusively for diabetic foot care specialists treating patients with chronic diabetes, foot ulcers, and related vascular and neuropathic complications. Generic telemedicine and OPD systems could not support the depth of structured assessment, diagnosis classification, and longitudinal tracking required in this domain.

The platform supports both online chat-based consultations and offline clinic workflows, while maintaining a single, centralized medical record for every patient across visits, doctors, and care settings.

Challenge

Diabetic foot care demands highly structured clinical assessment, precise diagnosis categorization, and continuity of care across multiple visits. Existing telemedicine and OPD systems were not designed for wound-based evaluation, bilateral foot assessment, or longitudinal tracking of high-risk patients.

We designed a specialized diabetic foot care platform that mirrors real-world clinical workflows and enables doctors to assess, diagnose, and manage patients consistently across both online and offline consultations.

What We Delivered: • Patient app with support for multiple profiles per user • Family-based appointment booking with relationship mapping • Pre-consultation structured health questionnaire covering pain and sensation, wound characteristics, ulcers and infection, mobility, footwear, comorbidities, and treatment history • Secure upload of wound images and medical reports • Online chat-based consultation with doctors • Doctor-side diagnosis classification including unilateral and bilateral involvement, risk stratification, DFU, DFI, cellulitis, osteomyelitis, neuropathy, dialysis, and amputation status • Prescription, report, and treatment plan sharing • Offline OPD workflows with nurse-assisted vitals capture (heart rate, SpO₂, random blood sugar) • Centralized MRN-based longitudinal patient record system • Unified tracking of patient progress over time
